Your First Appointment, Patient Forms, Map and Directions to Grand Rapids Office

We understand it's important that you feel comfortable with your choice of a dental office for your care. As with any new relationship, getting to know one another is the first step in determining whether or not our office will be a good 'fit' for you. Not all dentists are alike. They have different skills, different training, different technology, and different philosophies. Likewise, not all patients are the same. They too have different wants, needs, and desires.

We are dedicated to those who wish to keep their teeth for a lifetime in maximum comfort, aesthetics, and function. For those who have unfortunately lost their teeth, we provide comfortable and lifelike replacements.

Our office places an emphasis on understanding our patients' needs and desires, custom tailored individualized dental plans, comfortable treatment, predictable treatment, and running on time. Our focus is strictly on providing the best experience possible for our patients.


What To Expect
Your first visit to WestSide Dental involves a few special steps so that we can get to know you. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. We've also included a little background about our committed staff and what you can expect during the first visit procedures.

In preparation for your first appointment, there are several forms we ask that you fill out (links below!). We want your first visit to be more about getting to know you and less about paperwork, so we recommend printing the new patient forms and completing them in advance of your initial appointment. Simply download and  complete the forms, and then bring them with you to your first visit. If you would rather just fill the forms out at the office, we'll have them waiting for you, so please plan on arriving 15-20 minutes early.

Making sound decisions about your dental care and oral health is an easy thing to do with the right preparation beforehand: Make a list of questions to ask our office, so you don't forget anything on the day of your appointment. This includes any concerns you have, or oral problems you've been experiencing. If you have dental insurance, remember to bring your insurance card with you.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you.
